The Connected Care Halton Ontario Health Team (CCHOHT) is seeking Patient, Family, Caregiver Advisors.

Patients, families, caregivers play a vital role in building a seamless, connected health care system. As partners, they help guide not just their own care but also support their communities by planning, reviewing, and improving healthcare services.

Patient, family, caregiver advisors share valuable insights and experiences. Their input helps us design and deliver healthcare that meets the needs of our community and ensures people get the right care when they need it.
If you live or work in Oakville, Milton, or Halton Hills and have experience using healthcare services, or support someone as a family member, caregiver, or friend, we invite you to join our team to help shape the future of healthcare in our community.

The role requires a commitment of 3 to 5 hours per month on average. Meetings take place both in person and online. We very much appreciate the time Patient, Family, and Caregiver Advisors give. Recognition for Advisors is in accordance with the CCHOHT Recognition Policy.

The Connected Care Halton Ontario Health Team brings together healthcare and community service providers, as well as patients, families, caregivers from Halton Hills, Milton, and Oakville. Our goal is to connect care, support better health, create positive experiences for everyone, improve the healthcare system, and promote health equity.
