Connected Care Halton Ontario Health Team (CCHOHT) works with health and social services in Halton Hills, Milton, and Oakville to create a connected system of care.

We know that healthcare is at its best when it is designed with patients, families, caregivers, not just for them.

Those who have experience as patients, families, and caregivers in the healthcare system are key to creating a healthcare system that provides the right care, at the right time, in the way that works for them.

As partners in healthcare, patients, families, caregivers act as compasses not only in their own health decisions but for the communities they represent. They help to inform the planning, delivery, and implementation of changes in healthcare.

Family/Caregiver is defined as someone that the person with the health issues identifies as being of importance to their well-being and healthcare journey. They may be biologically, legally, or personally connected. They may include families, caregivers, friends, etc.

Patients - people who have used, use, or are potential users of healthcare services; persons with living and lived experience.
