The program provides employment and skills development, as well as mental health knowledge, self-awareness, and coping strategies.
The program has been developed by Mood Disorders Society of Canada (MDSC), in collaboration with Employment and Social Development Canada (ESDC).

Transitions to Communities aims to provide the direct supports needed to address the program participants emotional and physical challenges, with a focus on employability skills, mental health well-being and peer support.
During the seven weeks of vocational training, the participants will develop the specific skills and abilities to assist them in living satisfying, hopeful and meaningful lives.

Program Objectives:
- Increase mental health awareness, resiliency and coping strategies of the program participants
- Improve capacity for participants to maintain their well being
- Reduce risks of self-harm
- Develop community engagement opportunities for veterans
- Reduce risks of homelessness
- Eliminate barriers and assist persons with disabilities to obtain employment, continue with their education, or volunteer in the Calgary community
- Strengthen relationships between participants and their communities
